Children's Autism/Mental Health Case Management
Fraser Autism/Mental Health Case Management (Targeted Mental Health Case Management) helps families obtain and coordinate educational, health, mental-health, recreational, social, vocational and related services. Case managers will assist eligible individuals in gaining access to needed medical, social, educational, and other services to address their mental health needs.
Eligibility for case management services includes meeting county eligibility criteria and requires formal authorization through Hennepin County. Autism/Mental Health Case Management serves children and adolescents, 0-18, who reside in Hennepin County.
Locations: This program is currently offered in our Fraser Anoka, Fraser Bloomington, Fraser Eden Prairie, Fraser Minneapolis, Fraser Eagan and Fraser Richfield locations.
Payment/Funding: Medical Assistance and PMAPs will cover Autism/Mental Health Case Management for Hennepin County residents who meet eligibility criteria. For clients who have private insurance, Autism/Mental Health Case Management is funded through Hennepin County for eligible individuals.

Care Coordination is a similar service that may be available for individuals who do not live in Hennepin County. This is funded by some major commercial health insurance plans. We encourage you to call your insurance company to find out what your benefits are for these services if they have been recommended for you or your child.
