Waiver Case Management

Fraser Waiver Case Management services help people navigate the complex social service system.  Services include a comprehensive assessment of an individual’s strengths including their natural supports in addition to their healthcare, social, housing, educational, vocational and other service needs and preferences.

A person-centered service plan is developed with the client and their inter-disciplinary team. Case Managers assist the person in accessing the services identified in the plan and monitoring the ongoing quality and effectiveness of those services.  

Eligibility: Fraser provides case management services to individuals who are eligible for one of the following county-authorized waiver programs:

  • Developmental Disability (DD) Waiver
  • Community Alternatives for Individuals with Disabilities (CADI) Waiver
  • Brain Injury (BI) Waiver
  • Consumer-Directed Community Supports (CDCS) Waiver


Location: Services are available to individuals referred by Hennepin, Dakota or Ramsey counties.
