Case Management helps people access, coordinate and monitor needed services as they relate to the person’s assessed needs and preferences. The terms 'case management,' 'service coordination' and 'care coordination' are often used interchangeably.
Case management activities may include but are not limited to the following:
- Coordinate services
- Develop a service plan based on people’s assessed needs and preferences
- Evaluate and monitor services identified in the service plan
- Help people access needed services
- Help people identify potential service providers
- Inform people or their legal representative of service options

Waiver Case Management
Fraser Waiver Case Management services are designed to help individuals who have waiver funding navigate the complex social service system. Individuals using this service must reside in Hennepin County, Dakota County or Ramsey County. Click here for more information.
Developmental Disability (Non-Waiver) Case Management
Fraser Developmental Disability Case Management services are designed to help individuals who DO NOT have waiver funding navigate the complex social service system. Individuals must be referred by Hennepin County, Dakota County or Ramsey County to qualify. Click here for more information.
