Fraser Case Management helps individuals and families access and navigate complex, social and medical services
Fraser provides the following types of case management services to people of all ages in Dakota, Hennepin and Ramsey Counties.
- Brain Injury (BI) Waiver
- Community Access for Disability Inclusion (CADI) Waiver
- Developmental Disability (DD) Waiver
- DD Non-Waiver
You can be referred to Fraser following an assessment by your county of financial responsibility. After a referral, you are matched with a case manager, who will help you develop a service plan, tell you about different resources and services, help you connect to those services, evaluate the effectiveness of services and provide other needed support.
