Fraser Case Management is designed to help individuals and families access and navigate the complex social and medical services system.

Connecting You with the Right Resources

Case Management helps individuals with intellectual and development disabilities and those with mental health conditions gain access to needed resources. Our case managers connect people to medical, social, educational, vocational, financial and other necessary services that support their mental health.

Available to People of All Ages

Fraser provides the following types of case management services to people of all ages in Dakota, Hennepin and Ramsey Counties.

  • Brain Injury (BI) Waiver
  • Community Access for Disability Inclusion (CADI) Waiver
  • Developmental Disability (DD) Waiver
  • DD Non-Waiver
Help You Develop a Plan

You can be referred to Fraser following an assessment by your county of financial responsibility. After a referral, you are matched with a case manager.

Your case manager meets with you at least every four to six months. They help you develop a service plan, tell you about different resources and services, help you connect to those services, evaluate the effectiveness of services and provide other needed support. A case manager makes sure you have the resources you need to learn, work and live as an active member of the community.
