Around-the-Clock Personal Support

Fraser has 27 Supervised Living homes for adults with autism and disabilities throughout Hennepin and Anoka counties. These homes are available for individuals who need more support at home and in their community. Our goal for Supervised Living is to support the developmental, health, safety and social needs of each individual.

Staff

In each home, three or four people live together. Fraser has staff onsite whenever individuals are at home. They help individuals manage their daily needs, like doing their hair, brushing their teeth or exercising. Our staff works with each person to set and achieve goals. We also help them engage with the community through coffee dates, social events and trips to the supermarket.

A registered nurse provides a health service consultation. Accessible homes are available for individuals with physical disabilities.

Expense

The DD, BI or CADI waiver will cover the cost of the programming and housing. There is also a private pay option.

Available Openings


Goldenrod House is a split-level home located in Plymouth near Eagle Lake Regional Park. The individuals living there are between the ages of 52-63. When COVID restrictions do not prevent it, some of the residents attend a day program and others work in the community during the week. The individuals at Goldenrod are very social and enjoy activities such as puzzles, listening to music, basketball, going for walks, and going out to eat. The home is staffed 2:4 and offers awake overnight staffing.


Evelyn House

Evelyn is a rambler style home located in Edina. The individuals living there are in their late 30s-50s. They enjoy watching TV, doing puzzles, and coloring along with going out to eat, for a drive, or for a picnic at the park. On the weekends, some of the individuals go on outings with their families, but most of the time they are all home. The house currently offers 1:4 staff support, with awake overnight staffing. On evenings and weekends, there is often 2:4 staffing provided.


Sweet Briar

Sweet Briar is located in a quiet, residential neighborhood near Knollwood Mall. This is a rambler style home with one-level living. Currently, 3 males in their 20s-60s live at the Sweet Briar Home in Hopkins. The individuals enjoy accessing the community by going out to eat, watching TV, going to the movie theater, attending sporting events, and walking or going for bike rides in the neighborhood. The house has 1-2 staffing during awake hours and asleep overnight staffing. There are regular visits from a Registered Nurse.  Everything is located on the main floor of the home, except the laundry and food pantry which is located downstairs in the basement.


83rd St.

83rd St. is a rambler style home located in Bloomington with the kitchen, dining room, living room, and all 4 bedrooms located on the main floor. There are accessible entrances in the front and back of the home, with a patio set in the backyard.  The home is located in a busy residential area, conveniently located just a couple blocks away from Girard Lake Park. Currently, 3 females, in their 60's and 70's, live in the home. The individuals enjoy arts and crafts, watching TV, going out to eat, and going outside when it is nice out. The house has 1-2 staffing during awake hours and when individuals are home, and awake overnight staffing. There are regular visits from a Registered Nurse.
