Are you new to Fraser?
  1. For clients ages 0 to 17.5, please submit an electronic intake form. A team member will reach out to you to answer your questions and discuss next steps.
  2. For clients 17.5 and above, please call 612-767-7222 Monday-Friday anytime from 8:00AM-5:00PM. A team member will answer questions and discuss next steps.

FOR EXISTING CLIENTS

Do you have an upcoming appointment? If you are looking for making a change, please fill out Appointment Change Request Form and a representative will assist you with your request. Change request can include one of the following:

  • Convert to Telehealth
  • Ask for a Reschedule
  • Cancel your Appointment
